Primary radiotherapy of T1 squamous cell carcinoma of the larynx: Analysis of 478 patients treated from 1963 to 1985

Abstract

Radiotherapy was administered to 478 consecutively treated patients with laryngeal T1 squamous cell carcinoma between 1963–1985. One hundred and seventeen had a supraglottic, 358 a glottic, and 3 a subglottic tumor. Supraglottis: 71% males; 49% T1a; 14 patients with nodes. Glottis: 90% males; 82% T1a; 1 patient with node. The 10-year value for local control in the supraglottic group was 55% and in the glottic group 81%. No difference was observed between T1 a and T1b. Regional nodes and distant metastases were seldom seen in the glottic, but frequently observed in the supraglottic group. The treatment results appeared to be most favorable in women. The 10-year corrected survival for supraglottic and glottic tumors demonstrated a highly significant difference, 67% compared to 94%. There was a significantly increasing incidence of events with lower tumor differentiation. Split-course and conventional radiotherapy gave equal treatment results, but late complications were significantly more common with the former. A major problem was new primary cancers, which within 20 years occurred in 34% of patients surviving a supraglottic tumor and in 23% of the glottic patients. The predominant new site was the lung (23% and 13%, respectively). Thus, in the glottic group more patients died from the new cancer than from the glottic carcinoma.

In the United States it is estimated that there will be 51,540 new cases of head and neck cancer in 2018, accounting for 3% of all cancer cases and 1.5% of all cancer deaths. The three major risk factors for head and neck cancer are tobacco, human papillomavirus (HPV) infection, and alcohol. The treatment of head and neck cancer is dictated by the primary site. Most oropharyngeal cancers in the United States are now due to human papillomavirus (HPV) and as such have an improved prognosis. Early-stage oropharynx cancers can effectively be treated with surgery or RT. The standard of care for locally advanced disease is CRT; however, incorporation of transoral robotic surgery (TORS) is under active investigation. Surgery is generally considered the preferred initial treatment modality for oral cavity lesions with adjuvant RT with or without chemotherapy for patients with high-risk features on surgical pathology. In patients with laryngeal cancer, the goal of first-line therapy should be to preserve the function of the larynx, without sacrificing tumor control. Early-stage laryngeal cancer can effectively be treated with either surgery or RT. In early stage disease, the anatomic location and extent of disease will dictate if surgery is feasible. The treatment of choice in most locally advanced larynx cancers is concurrent chemotherapy and radiation. Management of head and neck cancers requires a multidisciplinary approach with effective integration of multiple specialties to achieve the desired goals of cure and functional organ preservation.

Outcome of T1N0M0 Squamous Cell Carcinoma of the Larynx Treated with Short-course Radiotherapy to a Total Dose of 50 Gy in 16 Fractions: the Birmingham Experience

To review the results of hypofractionated radiotherapy in T1N0M0 squamous cell carcinoma of the larynx.
A series of 100 patients treated with radiotherapy between 1993 and 2001 was reviewed. The median age was 67 years. The median follow-up was 7 years (range 3–14 years). Radiotherapy was delivered to a total dose of 50 Gy in 16 fractions treating daily, 5 days a week over 21 days.
Locoregional control rates with radiotherapy alone were 92% at 2 years and 88% at 5 years. After salvage surgery, the ultimate locoregional control rate was 96%. The relapse-free survival rates at 2 and 5 years were 85 and 70%, respectively. The cause-specific survival rates at 2 and 5 years were 99 and 97%, respectively. Overall survival rates at 2 and 5 years were 91 and 76%, respectively. Second primary cancers occurred in 21% of patients, primarily in the lung.
Radiotherapy to a total dose of 50 Gy in 16 fractions for T1N0M0 squamous cell carcinoma of the larynx offers high locoregional control rates with voice preservation. These results from a hypofractionated radiotherapy schedule are comparable with other longer fractionation schedules and offer potential for optimising resource usage.
Laser carbon dioxide cordectomy versus open surgery in the treatment of glottic carcinoma: Our results

To analyze oncologic results in patients with glottic cancers treated respectively, by laser CO₂ or open surgery, taking into account specific-disease survival, rate of locoregional recurrences, and their salvageability.
Retrospective study of 198 patients treated from January 1993 to June 2002 in the department of otorhinolaryngology at a Catholic university in Rome.
Glottic carcinoma were treated by laser CO₂ cordectomy in 132 patients (group 1) and by open surgery in 66 patients (group 2). The statistical analysis was performed by Kaplan Meyer method, log rank test, and χ² test.
The log-rank test points out significant differences between the 2 groups regarding specific-disease survival; no differences were found for disease-free survival. Within group 1, 16 patients developed local failure, which was retreated in 6 cases with laser surgery; in 9 (6.8%) with total laryngectomy, only 1 case was inoperable. In this group, 10 patients (62.5%) were salvaged. Within group 2, 18 patients developed local recurrences, which was retreated in 14 (21.21%) cases with total laryngectomy; the other 4 cases were not suitable for surgery. Of these 18, 8 patients (44.5%) were salvaged.
Our results show significant differences between the 2 groups concerning the specific-disease survival and the salvageability of local recurrences. In fact, in group 1 we found a higher salvage rate and a lower incidence of total laryngectomy. As already suggested, laser therapy leaves the laryngeal cartilaginous framework intact, avoiding the spread of the tumor out of laryngeal organ and resulting in a more favorable oncologic outcome.
